Please read and initial each acknowledgment:

I confirm that I or my child can swim independently without floatation devices, or I will remain within arm's reach of a responsible swimmer adult. Children under 14 must be accompanied by a parent or guardian at all times. One adult per two children is required. I will follow all posted pool rules and instructions from VIP Swim Club staff. Running on pool deck is prohibited. I understand that diving is not permitted in the pool area. Feet-first entry only. I understand that pool access may be suspended during thunderstorms or unsafe conditions at staff discretion. I will use pool equipment (toys, floats, noodles) responsibly and remove items when not in use.

Liability Release

Waiver of Liability: I understand that swimming and pool activities involve inherent risks including but not limited to drowning, injury, and property damage. I voluntarily assume all risks associated with pool use. I release the Homeowner, VIP Swim Club, their owners, employees, agents, officers, directors, and affiliates from any and all liability for injuries, damages, or losses that may occur during my participation in pool activities.

I have read, understand, and agree to the liability waiver above. I release the Homeowner and VIP Swim Club from any claims arising from my participation.
